TAX INFORMATION CHECKLIST

Note: This is a list of the most common items you might need to complete your return. Please make sure you compile these items prior to your appointment. Do not forget that it is your responsibility to come with your receipts already totaled. BRING IN A HARD COPY OF ALL DOCUMENTS.

PLEASE NOTE: ITEMS WE CAN USE ARE SUBJECT TO CHANGE THROUGHOUT THE SEASON AS THE LAWS CAN STILL CHANGE.

_____ 1. New clients must have copies of previous federal, state and local returns with depreciation schedules. (If applicable.)
_____ 2. Provide dependent social security number, date of birth, etc. (especially newborns)
_____ 3. Child care information with providers name, EIN or social security number, address and amount paid for each child.
_____ 4. Proof of medical insurance coverage for you, your spouse, and all dependents. If you receive the premium tax credit, you must provide the 1095A form. Please also bring your 1095 B or C if provided by your employer.
_____ 5. All W-2’s.
_____ 6. All 1099 forms received confirming income from interest, dividends, retirement, social security, disability, unemployment, gambling winnings, etc.
_____ 7. Adjustments to income, such as IRA contributions and student loan interest
_____ 8. If you sold any shares of stock or mutual funds, and basis information is not provided by the broker, detail all activity sold from original purchase date through date of sale. (Year-end summary statements are ideal.)
_____ 9. If you started or are continuing a business, compile all receipts for income, expenses, supplies, purchase of equipment, auto mileage, etc. (See the Schedule C Checklist.)
_____ 10. If you turned your house into a rental or purchased a new rental property, compile receipts for income, repairs, mortgage interest, property taxes, HOA fees, utilities, etc. (See the Schedule E Checklist.)
_____ 11. Copies of all LLC, Partnership, or Corporation K-1’s.
_____ 12. Medical receipts (including doctors, dentists, chiropractors, prescriptions, counseling, massage therapy, etc.) Do not forget to include medical miles for all medical events.
_____ 13. If you purchased, sold, or refinanced real estate, then you must provide an escrow/settlement statement for each transaction.
_____ 14. Car license receipts and sales contracts for the purchase of cars, trucks, motorcycles, RV’s, trailers, etc.
_____ 15. Year-end statement of mortgage interest (Form 1098), escrow activity, and real estate taxes paid.
_____ 16. Receipts for contributions to charities, both cash and non-cash as well as any charitable miles driven. (Goodwill, Salvation Army, etc.)
_____ 17. Copies of any federal or state tax correspondence received during the year, including all payments made or refunds received.
_____ 18. Please make sure you can provide the name of your bank, the routing number, and the bank account number for direct deposit. If you have a check, bring it, and you can use it to pay for your services.
_____ 19. Please have your driver’s license. If you have a spouse who can’t attend, we will need a photocopy of their driver’s license, as well.
_____ 20. Check, cash, or credit card as payment for services rendered. Payment is due at the appointment for completion of tax return. Please note that there is a $7.00 charge for credit/debit card payments. We encourage you to pay with cash or check to avoid this fee.
