Please total all receipts prior to appointment. We DO NOT need copies of your receipts. Please keep receipts with your tax documents for your own records. DO NOT staple any of your documents. Please remove all documents from envelopes.
This is a list of the most common items you may need to complete your tax return. Please note, we may need additional forms depending on your tax situation.

- Broker Packets/Stock Sales – Please include year-end summary and basis details, if not provided.
- Retirement
- Social Security
- Disability
- Unemployment
- Gambling Winnings
- HSA – 1099-SA
____3. K-1’s from partnerships, multi member LLCs and S Corp.
____4. Marketplace Health Insurance: Please provide form 1095-A.
____5. Student Loan Interest, IRA Contributions, and 529 Plan Contributions

____7. Rental Homes: Please use our Schedule E checklist to assist you in preparing your rental income and expenses. (Found via http://middletontaxservices.com)
____8. Child Care Information: Provider’s name, address and EIN or social security number and the amount paid for each child.
____9. Medical Receipt Totals: doctors, dentists, chiropractors, prescriptions, counseling, massage therapy, copays, lab fees, etc. We do not need a copy of your receipts. We do need the total amount spent on medical expenses.
____10. Medical Mileage: Please include the total miles driven to and from medical appointments.
____11. Real Estate: if you purchased, sold or refinanced a home, please include your Settlement Statement and 1099-S (if applicable) for each transaction. Please contact your title/escrow office to obtain these forms.
____12. Vehicle License Tax: This can be found on your vehicle registration or through your online MVD account. Please include for each vehicle owned. Vehicle license tax is NOT the total registration fee. (Include copy of sales contracts for the purchase of cars, trucks, motorcycles, RVs, trailers, etc.)
____13. Mortgage Interest & Property Tax: Please include form 1098 from your mortgage lender.
____14. Non-Cash Charitable Donation Amounts: i.e. Goodwill, Salvation Army, Helen’s Hope Chest, etc. We do not need receipts. Please include the total dollar amount donated. You can find donation valuations online. (Donation Value Guide: https://goodwillnne.org/donate/donation-value-guide/) If your donations are more than $250, additional information is necessary including purchase date, purchase amount, date donated and value at donation. The IRS requires taking and keeping pictures of any items you are deducting over $250.
____15. Cash Charitable Donations Amounts: Please total your cash charitable donations by organization and amount. Please note that “cash” can also be card, Venmo, Zelle, check, etc.
____16. Estimated Tax Payments: Please include the amount and date of any estimated tax payments made to the IRS or State agencies.
____17. IRS or State Correspondence: Please include any correspondence received.
____18. Bank Routing and Account: If you have changed your banking information, please update us.
____19. Payments can be made via cash, check, credit card or Venmo @MTS-AZ. Returns are not e-filed until payment is received.
____20. There will be a $5 fee for all printed tax returns.
____21 ***If you purchased a new car in 2025, and you are making payments on the car, please get us the amount you paid in interest and the VIN number for the new vehicle. There is now an “above-the-line” deduction. It can possibly be claimed even if you do not itemize. Certain requirements must be met in order to claim it:
- Must have been purchased in the 2025 calendar year.
- Vehicle must be new and for personal use. Used cars do not qualify.
- Must have a gross vehicle weight under 14,000 pounds.
- Must have a loan from a qualified lender.
- Modified Adjusted Gross Income limits do apply.
- This law ends at the end of 2028.

____22. Energy Credits: If you have replaced doors, windows, AC, insulation, installed solar, etc., please talk with your tax preparer to see if the upgrades qualify for energy credits. Please submit your invoice with your documents.
If there was a Sale of Rental, Short Sale, Foreclosure, or Bankruptcy and your home was taken, please let us know before your appointment, because much more information will be needed to complete your return.
Large stock transactions also take more time, and WE NEED your broker packets at least one week in advance of your appointment.
