2021 TAX INFORMATION CHECKLIST
Please total all receipts prior to appointment. We DO NOT need copies of your receipts. Please keep receipts with your tax documents for your own records.
This is a list of the most common items you may need to complete your tax return. Please note, we may need additional forms depending on your tax situation.

____2. All 1099 forms received confirming income, including:
- Interest
- Dividends
- Broker Packets/Stock Sales – Please include year-end summary and basis details, if not provided.
- Retirement
- Social Security
- Disability
- Unemployment
- Gambling Winnings
- HSA – 1099-SA
____3. Please provide official IRS letter with the total amount received for the 3rd stimulus payment. IRS 6475
____4. Please provide official IRS letter with the total amount received for the Advance Child Tax Credit. IRS 6419
____5. K-1’s from partnerships, multi member LLCs and S-Corp.
____6. Marketplace Health Insurance: Please provide form 1095-A.
____7. Student Loan Interest and IRA Contributions
____8. Business: Please use our Schedule C checklist to assist you in preparing and totaling your income and expenses.

____10. Child Care Information: Provider’s name, address and EIN or social security number and the amount paid for each
child.
____11. Medical Receipt Totals: doctors, dentists, chiropractors, prescriptions, counseling, massage therapy, copays, lab fees,
etc. We do not need a copy of your receipts. We do need the total amount spent on medical expenses.
____12. Medical Mileage: Please include the total miles driven to and from medical appointments.
____13. Real Estate: if you purchased, sold or refinanced a home, please include your settlement statement for each
transaction.
____14. Vehicle License Tax: This can be found on your vehicle registration. Please include for each vehicle owned.
(Include copy of sales contracts for the purchase of cars, trucks, motorcycles, RVs, trailers, etc.)
____15. Mortgage Interest & Property Tax: Please include form 1098 from your mortgage lender.
____16. Non-Cash Charitable Donation Amounts: i.e. Goodwill, Salvation Army, Helen’s Hope Chest, etc. We do not need
receipts. Please include the total dollar amount donated. You can find donation valuations online.

____17. Cash Charitable Donation Amounts: Please total your cash charitable donations by organization and amount.
____18. Estimated Tax Payments: Please include the amount of payments made to the IRS or State agencies.
____19. IRS or State Correspondence: Please include any correspondence received.
____20. Bank Routing and Account: If you have changed your banking information, please update us.
____21. Payments can be made via cash, check, credit card or Venmo @MTS-AZ. Returns are not e-filed until
payment is received.
____22. There will be a $5 fee for all printed tax returns.
New Clients:
- Please provide full names, DOBs, SSNs for taxpayer, spouse and all dependents.
- Please provide complete copies of your previous year’s federal, state and local returns.
- Please provide us a copy of you and your spouse’s driver’s license.
